McKenzie-Willamette’s Case Management Team integrates the functions of Discharge Planning and Resource Management into the role of Nurse Case Managers.

Nurse Case Managers provide assessment and multi-disciplined collaboration in assessing medical records and resource management for individual patients. They are dedicated to helping each patient return to the community with the least amount of disruption to their everyday life, arranging appropriate discharge for continued care and treatment. Through proper patient evaluation, proactive planning is implemented to ensure patients receive needed services in the most appropriate setting, using necessary resources.
