If You Have Health Insurance
All patients should familiarize themselves with the terms of their insurance coverage. This will help you understand the hospital's billing procedures and charges.
A member of the Pre-Access Department will contact you prior to your admission to gather insurance information.
We will need a copy of your insurance and identification cards at the time of admission. You will be asked to sign a Condition of Services Rendered form which assigns benefits from the insurance company directly to the hospital.
If You Are a Member of an HMO or PPO
Your plan may have special requirements, such as pre-certification (also known as prior authorization), for certain tests, procedures or surgery. It is your responsibility to make sure the requirements of your plan have been met. If your plan's requirements are not followed, you many be financially responsible for all or part of the services rendered in the hospital. Some physician specialists may not participate in your healthcare plan and their services may not be covered.
If You Are Covered by Medicare
We will need a copy of your Medicare card to verify eligibility and process your Medicare claim. You should be aware that the Medicare program specifically excludes payment for certain items and services, such as cosmetic surgery, some oral surgery procedures, personal comfort items, some lab tests and others. Deductibles and co-payments also are the responsibility of the patient.
If You Are Covered by Medicaid
Medicaid also has payment limitations on a number of services and items. Medicaid coverage will depend on the plan assigned to you. Please be aware that the LIPA Standard option does not cover elective hospital procedures.
If You Have No Insurance
A representative from the Financial Counseling Office is available to discuss financial arrangements with you. Please contact them at: 541-744-6183.
If you need assistance in applying for Medicaid or other governmental assistance programs, please contact Eiligibility Services at: 541-744-8437.
Professional Services
You may receive bills from physicians you did not see in person during your hospital stay. These physicians could be pathologists, radiologists, cardiologists, anesthesiologists and other specialists that were diagnosing and interpreting test results while you were a patient.
These bills are for professional services rendered by these doctors in performance of these services. Theses physicians are required to submit separate bills to your insurance company. If you have questions about the physician bills, please call the number printed on the statement you received from them.
