At McKenzie-Willamette Medical Center, we're here to help you heal—and to make certain your experience with us is as safe, restful and restorative as possible. However, before you visit us, there are a few things you might want to know about our campus and making the most of your hospital stay.
Accessing Medical Records:
Normally, Medical Records are requested in person, however, because visitor restrictions are now in place at this hospital, you can request medical records by downloading this Release of Information form, faxing it to 541-741-7219 or mail to: McKenzie-Willamette Medical Center, 1460 G Street, Springfield, OR 97477, ATTN: Medical Records
Pre-Admission Testing:
Your PAT appointment is very important. Please take a moment to print out the PAT Education and PAT Health History forms. The Health History form will need to be completed before arrival for your appointment.
