Medical Records

Medical records can be requested in person or by fax. Please complete the Release of Information Form and either bring it with you to the hospital or fax it. MWMC Medical Records Department is open Monday – Friday 8:30am – 4:30pm or you can fax the completed form to 866-877-5039.
